The Department of Agriculture’s Retail Food Program requires the following plans to be submitted for review; Floor Plan, Plumbing Plan, Equipment Plan(including cut sheets), and a Finish Plan(materials used on your floors, walls, and ceilings). The Retail Food Program has 7-10 days to complete the building plan reviews upon receipt of the required sheets.

- A Completed and signed license application (The owner or corporate officer must sign this application) (Attached)
- 서명이 완료된 라이센스 신청서(비즈니스 오너 또는 임원이 신청서에 서명해야함)
- A Completed Universal Business Plan Form (Attached) Note: Only use the mobile Business plan if you have a mobile vehicle).

- A Completed and Notarized Affidavit (S&V) of Lawful Presence within United States. On the first line of the affidavit write, “Food License’’. On the second line write, “GDOA’’ or ”Ga. Dept. of Agriculture’’. On the line for secure and verifiable document write, “Driver’s License; Passport’’, etc. for the document you will provide with the affidavit)
- Verification of Lawful Presence Within United States (O.C.G.A. § 50-36-1)
- A clear color copy of your Photo ID (Government issued identification only: Passport, Driver’s License, etc.) (Please Attach and send along with the 0ther documents)
- Private Water Sources: (If your firm is on a private well, you will have to have your water tested and a negative lab result will need to be submitted to GDA)
